If you are a company or an organization ("legal entity requestor") making a request on behalf of a California resident, please follow the process below.

Please note that you will be required to provide the following documents in order for us to process your request:

1. A copy of the data subject's signed permission authorizing you to submit a request on their behalf; or

2. A copy of a document to confirm you are authorized to make this request. We will accept a copy of power of attorney, legal guardianship, or conservatorship order.
